Implement Navigation System
Many families struggle to access programs and services that could improve the healthy development of their children. Providers also face challenges in connecting families with services. How can we ensure that ALL families with young children get the services they need, when they need them?
Teams are working together to design, test and implement a system to help families navigate the wide variety of programs available for families in Guilford County. We begin by meeting with parents and children at five key points — during pregnancy, within a few weeks of coming home from the hospital, and when the child reaches 1 year, 2 years, and 3 years old. All families, regardless of background or income, can use additional support during this critical time of development, so we will meet with all families to learn what they want and need.
The navigation system will eventually expand beyond age three.
